4-H Incident Report Form Policy
This procedure furnishes guidelines for completing a 4-H Incident Report
Form for violations of the 4-H Code of Conduct.
When any violation of the 4-H Code of Conduct is reported, the
Activity/Event Coordinator must complete the 4-H Incident Report Form. The
Activity/Event Coordinator must fill out the form completely and include the
nature of the violation, when the incident occurred, findings of facts
determined, and disciplinary action taken, if any.
In reporting the finding of facts, the Activity/Event Coordinator should
fully explain in a written, narrative form all of the relevant information
gathered during the investigation of the incident. The narrative must be
very thorough and detailed. Findings of facts are determinations made by the
Activity/Event Coordinator based on the information available to him or her.
Any witnesses that were questioned must also be listed on the form.
When any violation of the 4-H Code of Conduct is reported, the
Activity/Event Coordinator must complete the 4-H Incident Report Form.
Section III must be completed if the violation of the Code of Conduct does
not require for a Review Board to be convened. If a Review Board is
required, Section III must be left blank.
Distribution of completed Incident Reports should be as follows:
Send Original to: Appropriate County Staff Chair
Send Copies to:
Appropriate District Director
Assistant Director - 4-H Youth Development
Parent(s) of the 4-H member
Activity/Event Coordinator File
Review Board (if convened)
